Enrollment and payment
One player. Six months. A clear commitment from both sides.
This agreement is between Lee Green Basketball LLC, operating The Player Gap (TPG), and the parent or legal guardian below. The signing adult is responsible for payment. The child is the player receiving services.

Payment due dates
This agreement does not charge your card or bank. Payment instructions come separately, and any automatic payment setup needs its own authorization.
The work we agree to do
Coaching, practice, feedback, and a shared schedule.
3. What the fee includes
Court sessions. Twelve shared 60-minute sessions over six months, normally two each month. Sessions are with other players. Individual goals guide the work.
Assessment and plan. An individual TPG 360-degree Assessment and Player Gap Report. This work takes place within the first session and first monthly review.
Practice and film. One monthly plan with drills, targets, and a simple log, plus two film tasks each month. TPG provides the study clips and teaches the player how to use them.
Monthly video feedback. One submission each month with up to five minutes of drill or game clips and a short reflection. TPG gives one focused written or voice response. Full games are not required.
Updates and reviews. One brief parent update each month and progress reviews at months three and six. Reviews take place within the scheduled sessions and feedback.
Separate labs, camps, extra private sessions, and extra video reviews cost extra and need your approval first. Any free extra session is at the coach's choice and is not promised.
4. Player effort and parent support
Plan for about 30-45 minutes of work per week outside court sessions, with some weeks lighter. The player should do the agreed drills and film work, keep the log, and bring questions. The parent helps with the routine, transport, reminders, and short recordings as needed.
TPG will adjust tasks to the player's needs and team schedule. Save coaching questions for sessions and the monthly review. Progress depends on effort and other factors. No set results, playing time, team spot, scholarship, or recruiting offer is promised.
5. Schedule and monthly deadlines

Read these terms before you commit to the full six months.
6. Payments and missed sessions
Ordinary withdrawal. Losing interest, joining another team, routine schedule conflicts, choosing a different trainer, or deciding to stop are not, by themselves, grounds to cancel the fee. If you leave for these reasons, unpaid installments remain due on the dates listed. Fees already paid are not refunded for ordinary withdrawal. This applies while TPG keeps the agreed services available, subject to section 7 and any rights required by law.
Occasional absence. Tell TPG as soon as you reasonably can about illness or a family conflict. The group session will still take place. TPG will account for missed work in the player's tasks and next scheduled session. A separate private makeup, refund, or fee credit is not included. The payment schedule stays the same.
Late or failed payment. TPG will send written notice of an overdue payment. Please resolve it within seven calendar days. A missed payment or canceled payment method does not end this agreement. This form does not authorize automatic bank or card charges. Any automatic payment setup requires a separate authorization.
7. When the remaining commitment may end
Serious hardship. If a serious injury, extended illness, or major family event makes it impossible to continue, send TPG a written request. We will review the request in good faith. If the condition is met, we will confirm an agreed end date in writing. Payments for the remaining months after that date will stop. Any prepaid amount for those months will be refunded. Each full remaining program month is valued at $625.
A request alone does not change the due dates, unless the law provides otherwise. TPG may ask for reasonable proof of the need to stop. A brief statement is enough to begin the review; do not send detailed medical records. We will confirm any remaining balance or refund in writing. Any refund owed will be sent within 30 days of the agreed end date, or sooner if the law requires.
If TPG changes or ends the program. If TPG ends service early, or makes a lasting change to the agreed time or location that you cannot reasonably attend, you may end the remaining term. Future fees will stop and prepaid unused fees will be refunded. For this purpose, a full month is $625; a partial month is based on the unused calendar days in that program month.
8. Coach cancellations and concerns
If TPG cancels a court session, TPG will offer a makeup at no added charge and work with you on a reasonable date. If no makeup can be agreed, TPG will give a fair fee credit or refund for the missed service and confirm the amount in writing.
Tell TPG in writing about missing services or other concerns. We will work to address them. These terms do not remove any right to cancel, receive a refund, or seek other relief under the law.

9. Other terms
Follow safety rules and treat others with respect. Tell the coach about limits on safe play. Stop and tell the coach if pain or injury occurs. This form covers services and payment. It does not waive injury claims or grant permission to use the player's image in ads.
The term ends on the program end date in step 1. There is no automatic renewal. Continuing requires a new agreement. This form and any signed schedule or changes set the terms. Both sides must agree in writing to changes in the fee, term, or services. Texas law and other laws that must apply govern this form. A term that cannot be enforced will not remove your legal rights.

11. Electronic consent and signatures
Each signer agrees to use electronic records and intends the typed name below to serve as a signature. You may ask for a paper copy or sign on paper instead. You must be able to open, save, and print a PDF. TPG will email a full signed copy to the parent.
